　　Duke and Yan, two long time rivals, are competing in the world’s most dangerous underground fighting tournament, where powerful crimelords can gamble and influence who they fight, the terrain they fight on and what weapons will be used. 記者Bob及Alan在采訪時在一大廈電梯內遇鬼，Bob全身突然腐爛并被吊死，Alan則被嚇瘋。CID幫辦阿澤（古天樂）調查此案，見一名紅衣女郎（黎姿）突然失蹤。他們發現Bob死前拍下一張白朦朦的相片，其中的影像不清，追查之后才知與之前的紅衣少女吊頸命案有關。同時，阿澤見手上被Alan爪傷的爛肉開始擴散，驚惶不已。而Alan亦在醫院中全身潰爛而死。原來紅衣少女死前是一位著名模特兒，她被傳媒發現和一名富商有染后離奇墮樓而死。當時是攝記的Bob和十二少一直跟蹤她并目擊案發經過，兩人不但見死不救，還拍下少女死狀恐怖的照片，不顧私隱地大做文章。女鬼向周刊老板和記者追命，四人相繼恐怖地被紅傘插死。阿澤追查此案中，勾起一段失意的往事——他自己亦與紅衣女跳樓有關聯，因此成為女鬼復仇的下一個目標......
